PHR's Terms of Use
1. The software application MY HEALTH ANGELS - PERSONAL HEALTH RECORD (hereinafter "PHR Program") is owned by «APOLLO Advanced Health Technologies Commercial & Electronic Applications SA" with distinctive title "APOLLO SA". Legitimate holder of all intellectual and industrial property rights, including patent rights, rights for trademarks and distinctive signs, Software Application and the overall content, is the property of APOLLO SA. Any attempted violation of such rights, is explicitly prohibited, including and without limitation, attempting to copy the software in whole or in part, de-compress, attempted acquisition of the source code, reverse engineering and so on. The user of «PHR Program" receives a personal, nontransferable license to use of the «PHR Program" under the relevant Convention between the Health provider (doctor or any other) and the provider-supplier of the Application «MY HEALTH ANGELS».
2. The personal data and sensitive personal health data that are included by the users of the application My Health Angels - Personal Health Record and are contained therein ("Data"), are kept strictly confidential and private and are protected by the provisions of the Greek legislation on the protection of personal data, medical confidentiality and the Code of Medical Ethics.
3. The Data kept in Personal File / Sensitive Data held by the Doctor or Health Care organization (including the persons they have designated for the lawful processing of such data), from which you received the license and passwords for access and use of the «PHR Program" (hereinafter "Doctor or Health Care Organization"), are responsible for data processing, in accordance with current legislation and the terms of use of «PHR Program."
4. Data are accessed only by the user and the treating Doctor or Health Care organization (except in simple data, which are accessible to APOLLO, in accordance with the terms of use and privacy policy). The PHR program gives you, however, the ability to send Data to third parties of your choice.
5. It should be taken into account that always there is a danger that these data are incomplete, inaccurate, obsolete, not updated, and therefore may have just informative character, and not offered in any way to establish, support and / or justify any medical diagnosis, clinical examination, prescription and/or administer a medical opinion of any kind. To diagnose and / or administer a treatment, the physician according to the law and Code of Medical Ethics must conduct a personal and direct diagnosis.
6. User names and Passwords in PHR Program are known only to you, and we recommend keeping them confidential, to prevent third parties access to your data. If you notice that your passwords are no longer safe, immediately contact APOLLO to cancel / replace them.
7. The PHR Program in any case is not suitable for file storage purposes and therefore it is recommend that you keep copies of personal data in your own storage media.
8. To PHR Program is an inseparable part of the Unified Application «MY HEALTH ANGELS» which the Doctor or Health Care organization uses under the relevant contract with a supplier of their choice. Passwords remain active, only for the whole period mentioned conditions of use of Apollo.
9. The availability of the PHR program is not guaranteed.
